In loving memory of Philip Richard Jai Moffatt who sadly passed away on 10th July 2020
MOFFATT Phillip Richard Jai “Phill”
Originally from Moe Victoria and late of Deagon Queensland.
Phill passed away in the Prince Charles Hospital on Friday 10th July 2020 at the time of his passing he was just 32 years of age.
Phill leaves behind his loving parents, Anne, Karen and Nicky along with his 6 siblings and many nieces and nephews.
He will always be remembered as a loving, caring person with a positive outlook, no matter the circumstance. He embodied that all through his life and did so right until the very end.
Phill had a passion for the arts and was an exceptional darts player, poker player and bowler where he made incredible friendships along the way.
Thanks go to the Doctors, Nurses and Support staff of “The Prince Charles Hospital” Palliative care unit for their services.
Due to current number restrictions the service will be live streamed for those who cannot attend.
The service will be held on Tuesday 21st July 2020 commencing at 2:00pm, a link will be provided.
After the ceremony Phill will be taken for a private cremation.
As per his wishes he will be taken to Adelaide where a service will be held at a later date as his final resting place.
